Abstract

|
Isolated trochlear nerve palsy due to dural carotid-cavernous fistula (CCF) is extremely rare. We report a case of trochlear nerve palsy caused by a dural CCF, which was successfully treated by transvenous embolization. A 62-year-old man presented with vertical diplopia. On neurologic examination, we discovered a trochlear nerve palsy on his left eye, but did not detect any abnormalities in other tests including brain imaging. Over the year, the patient¡¯s symptoms continued to worsen. Eventually, an orbital CT with angiography revealed a dural CCF involving his left side.
